AFTER performing the instructions found on the previous page, click on the numbered selection that best describes your event(s).

1. Public - SINGLE DATE  -- an event open to the general public, where the artist is to perform one or more sets on a single date, and where the artist's performance is to be advertised in the public media, such as television, radio, newspapers, etc..

2. Private - SINGLE DATE -- an event/function not open to the general public, not advertised using general public media, such as television, radio, newspapers, etc.. Artist is to perform one or more sets on a single date. The event is to be attended by invitation only by a specific group of people, such as association members, club members, company employees, trade show attendees, or students, etc. (The type of event is typically promoted privately, such as through word-of-mouth, flyers, letters, company or school newsletter, etc.)

3. Public - MULTIPLE DATES -- public events where the artist is to perform on more than one date.

4. Private - MULTIPLE DATES -- private (non-advertised) functions where the artist is to perform on more than one date.

5. Special request -- hiring an artist for special situations such as singing the national anthem, or making a "non-performance" appearance, such as for autograph signing, meet and greet, attending an "after party," etc.


Definitions:

Public -- if open to the general public and the artist's performance is to be advertised in the public media (such as television, radio, newspapers).
Private -- if not open to or advertised to the general public. For example, attendance is by invitation only or is restricted to a specific group, and the event is to be promoted privately such as by word of mouth, trade paper, company or school paper, bulletins, letters, flyers, etc.


If you are hiring an artist to perform multiple times, and at least one of the performances is being advertised to the general public, use the PUBLIC form.
